Handling fee | Please enclose a postal money order for 600 yen (tax included) as a handling fee. * Postal money orders can be purchased at post offices. |
Submission methods | After completing the required items on Yamaha's Personal Information Disclosure Request Form, please mail them to the address below together with the other required documents. As a rule, we will not return the handling fee in such cases. |

Notes | Information will not be disclosed if any of the following have been determined to be the case. When information is not to be disclosed, Yamaha will notify the requester of that fact and the reason for that decision. As a rule, we will not return the handling fee even in cases when the information is not disclosed.
